Expiration Date of the Standard Flood Hazard Determination Form (SFHDF)

The current Standard Flood Hazard Determination Form (OMB Control No. 1660-0040) has an expiration date of 09-30-2023. However, please be aware that FEMA has extended the use of the form indefinitely, pending an additional review and comment period.

For compliance purposes, the current version of the form is still valid and may continue to be used until further notice. On October 10, 2023, FEMA updated the message for the Standard Flood Hazard Determination Form, found at the below link for the National Flood Insurance Program Underwriting Forms.
